- Runway incursions and excursions
- Controlled flight into terrain
- Midair collisions
- Loss of control–in flight
- Turbulence
- System component failure–non-powerplant
- Abnormal runway contact
- Insufficient funding for modernization
- Shortages of qualified personnel
- Poor safety data collection systems
Some of these items have been on the list since the inception of flight, while others have come and gone with the introduction of new technologies and systematic approach to reducing negative effects of human error in the aerospace system.
Imagine if the Wright Brothers and Charles Taylor could have seen this engine?
We've come a long way, especially with the affordability and capabilities of flight simulators.
